PDF Version of Adult Referral Application Package

Intake process:

The potential client will need to obtain a 'referral worker/counsellor' who will be available to work with them both 'pre' and 'post' treatment. Together they will fill out the intake package and then the client will need to go in for a medical examination and TB Testing which will require their physician to fill out a further section of their intake package.

When all is completed they send it in to our treatment centre, via fax, or email.

Our Intake Worker will then look through the package and see if everything is completed and looking for any obvious items that the Review Panel may flag. If there is anything out of order or incomplete, they will send it back to the Referral Worker in the form of a 'Confirmation of Receipt' that will note anything that needs further clarification or completion.

The Confirmation of Receipt will also provide the 'cut-off date' and 'time for the package to be completed' in order to be considered for the next Intake Program (this is 2 Wednesdays@ 14:00 before Intake Starting Date.)

The Referral Worker will continue to work with the Client until all those requirement have been met. After the Intake Worker has received in writing all the completed information, they will send the potential client's file to a Review Panel- (made up of 3 Program Counsellors) who will look the file over and deem whether this client would be a good fit for our Program at this time. They will give their findings to the Intake Worker, which may deem them to work further with the Referral Worker and Client to
clarify or add additional information.

If all is accepted, at 'this time the Intake Worker will send out an acceptance letter to the Referral Worker with additional recommendations for the client to follow prior to attending treatment.

In the event that they were not accepted, the Intake Worker will draft a letter to the Referral Worker indicating this and giving reasons as to why this was the case. In the event that we have received the sufficient numbers of clients to fill our Program
quota, additional completed and approved client files will be placed on a waiting list to be called in the event that one or more of the accepted clients do not show for program.

Some of the common things that are flagged in the looking over the package might include:

Incompletion of the Intake Package and/or the Medical portion of Intake Package
Literacy concerns - It is necessary that the client be able to read and write and comprehend at a Grade 8 Level, as there is much reading that the clients are expected to do our their own, We do not want to further frustrate or set people up to fail if they are not able to do this.
Legal Documentation not submitted - We require a copy of all Bail, Probation, Parole Orders, including knowledge of court dates and nature of charges.
